How do I access the programs at Cancer Services?
 
Simply call and make an appointment to meet with an advocate. An initial “get acquainted” meeting will help you understand everything available and give us a chance to understand how we can best help you and your family during this time. You will complete a short information form and spend 30 minutes or more with a Cancer Services advocate.
 
Our services are available to all regardless of their ability to pay. From this time on we’re here to help in whatever way you need.

We are able to do what we do through the generous support of individual donors, local corporations, charitable foundations and United Way. We understand the impact that a cancer diagnosis makes on a family and want to help.
 
 
Requirements to become a client of Cancer Services of Northeast Indiana:
Have a diagnosis of cancer or brain tumor.

Live in one of the following counties: Allen, Adams, DeKalb, Huntington, Kosciusko, Lagrange, Noble, Steuben, Wabash, Wells, or Whitley 

Complete Client Intake form and Consent for Services Form when you meet with your personal advocate. Walk-ins are welcome; however appointments are encouraged. You are welcome to prepare the form prior to your meeting.  You can do that online now

Meet with a Client Advocate who will assess needs and provide support and information. Intake hours are 9a – 4p. Agency is open 9a – 5p for all other services.

If the person with cancer is unable to come to Cancer Services, a family member or friend may register her/him.  We do ask that the consent form be signed by the person with cancer. Special arrangements can be made for home visits. Please call if you have questions:
